When calling, please have the following information available:

1. Name of sending facility/unit and call back number
2. Patient name, gender, and DOB
3. Diagnosis
4. Reason for transferring patient
   • Higher level of care
   • Service not provided at sending facility
   • Continuity of care (provider and last visit with provider)
5. Contact number for the physician requesting the transfer (direct line or cell phone only, no pagers)
6. Insurance information for non-emergent transfers
7. Type of bed requested
